Register With The Practice?

New policy states we have to obtain two forms of identification from anyone who wishes to join our practice. These are:

1. Photographic evidence of identification.
2. Utility bill or some other evidence of current address.

New Patients

It is now our policy when registering with Ringwood Medical Centre to obtain 2 forms of identification from every patient wishing to join our practice. It is necessary for all new patients wanting to register with the practice to book an appointment for health screening examination and to complete a health questionnaire.

We regret that we are unable to accept new patients outside the area indicated on our map.
