Register With The Practice?

Our policy is that we have to obtain two forms of identification from anyone who wishes to join our practice. These are:

1. Photographic evidence of identification.

2. Recent utility bill or some other evidence of current address.

It will be necessary for all new patients to book an appointment for a health screening examination and to complete a health questionnaire.

We regret that we are unable to accept new patients outside the area indicated on our map.
