Change My Personal Details?
Please advise reception if you change your name, address or telephone number.
It would be a great help if you would also ensure that we have your telephone
number and your postcode. If you are moving home and will fall outside of the
practice boundary (as indicated on the back cover of this booklet) you will be
required to register with a GP in that area, as soon as possible
